FORM 472 (BEAR)

FORM 498
If you ever have to file a BEAR and have USAC reimburse you, you MUST have a 498 on file for direct deposit.

- An up-to-date SAM.gov account and UEI are required
- NEW: You now upload your banking information in the EPC portal during the Form 498 creation process. You’ll upload one of the following:
- A cancelled check OR
- A letter from your bank or the first page of a banking statement that clearly indicates the name of the entity, bank name, routing number and account number. On the letter, it is also recommended to include name and address of the school, name and title of the person requesting (school administrator, finance contact).
